Background

Patient Pattern's cloud software Care Coach had begun as a platform to help payers and providers use the emerging practice of Frailty to help deliver greater care for geriatric patients. Through working with their customers, they discovered that the tool could expand to some of their biggest challenges that no single software in the market has addressed.

Solution

A redesigned software solution that is aligned with their ultimate vision of greatly improving patient care. We identified and solved for their customer's biggest pain points while setting the company up for better financial outcomes. The redesign incorporated several new innovative features and was done efficiently while creating their first ever Design System in Figma.

 

 

We conducted an extensive and robust discovery phase to dive deeper into the current product and identify overall challenges in the industry.

 

Discovery

We identified key strategies to improve the product experience and encourage behavior that would prove better clinical outcomes. Through competitive research, user journeys, feature exploration, and in-depth analysis, we conducted a UX audit of the “Care Coach” web app. This audit delivered best practices and actionable recommendations, uncovering new opportunities to improve the platform.

 

Dashboard

We designed the dashboard for healthcare providers to offer a clear overview of progress, tasks, and key metrics. By prioritizing intuitive visuals and organized task management, we help providers quickly identify next steps, track goals over time, and focus on improving patient care.
